Outpatient Services
Outpatient services are designed to help individuals with
diabetes improve their health status and quality of life,
and reduce the risks for diabetes-related complications. Our
twelve month program:
- Educates and motivates you to practice necessary self-management
behaviors through:
- A core education series that provides basic skills
and knowledge recommended by the American Diabetes Association.
- Advanced education modules that focus on specific
needs, such as weight management, insulin administration,
oral medications, and exercise.
- Support groups that offer additional diabetes management
education, as well as the opportunity to share experiences
with other individuals who have diabetes.
- Specialized education programs, including diabetes
and pregnancy.
- Encourages you to keep regular appointments with your
physician.
- Periodically assesses your progress, such as reductions
in Hemoglobin A1c levels and improvements in
self-management behavior changes.
- Provides your physician with regular reports detailing
your status with progress towards goals.
- Is covered all, or in part, by most insurance companies,
including Medicare.
