Outpatient Services

Outpatient services are designed to help individuals with diabetes improve their health status and quality of life, and reduce the risks for diabetes-related complications. Our twelve month program:

  • Educates and motivates you to practice necessary self-management behaviors through:
  1. A core education series that provides basic skills and knowledge recommended by the American Diabetes Association.
  2. Advanced education modules that focus on specific needs, such as weight management, insulin administration, oral medications, and exercise.
  3. Support groups that offer additional diabetes management education, as well as the opportunity to share experiences with other individuals who have diabetes.
  4. Specialized education programs, including diabetes and pregnancy.
  • Encourages you to keep regular appointments with your physician.
  • Periodically assesses your progress, such as reductions in Hemoglobin A1c levels and improvements in self-management behavior changes.
  • Provides your physician with regular reports detailing your status with progress towards goals.
  • Is covered all, or in part, by most insurance companies, including Medicare.
